966-I1
A Class Disposable personnel breached the book, telling others it’s fascinating, and telling them it smells good, for then to try it and die. This incident 966-I1 killed about 12 to 23 people. Because of incidents like 966-I1, 966 may soon become Arcane if in the wrong hands.
014-I1
After her couple of years in containment, Anomaly 014 expressed her desire to research more about the Paleolithic era and learn more about "cavemen" and "primitive men". To complement the research and fulfill her wish, she was informed about Anomaly 013 ("Hardy") and that he was basically a caveman. Anomaly 014 was thrilled. A few moments later a cross test was done between the two anomalies in the enclosure of 013. Everything went smoothly, as expected, 013 was wary of 014, while 014 kept approaching and analyzing him. When 013 understood that, indeed, 014 was a cow, he pounced on it with a rudimentary spear (this spear seems to have been created in secret by 013 at some point during his stay in containment), and ended 014's life, slitting its throat and quartering it in a short time; after which, he began to eat its flesh.
Anomaly 013 was subdued.
Our Big Incidents
050-I1
Test subject D-3765 was sent into the containment cell. In the first 10 minutes, everything was normal. D-3765 was sent into his cell. About half a hour later, there was a CD riot in Sector A, with any CD near the riot joining the riot, including infected D-3765. After Task Force 050 contained the breach, the dead bodies were researched, and burnt. The area was sanitised, and kept locked for 10 days.
223-I1
During a test, multiple CD raided the chamber and breached 223. They got some of 223-D1 (Device 1 made for Anomaly 223) headphones so they won’t be ear-blasted. They managed to keep it hidden until lunch-time, when they absolutely obliterated $1/3$ of the facility personnel, or all the personnel in the cafeteria. Overall, casualties were 40+. It remains one of the biggest incidents.
